Required Documents for Enrollment

To begin the process, please complete our Pre-Registration 2025–26 form. All students enrolling at ACHS must provide the following documents, as required by APS:

• Proof of Residency
• Birth Certificate
• Immunization Records
• Previous School Records (if applicable)
• Parent/Guardian Identification

Next Steps

After submitting your pre-registration and required documents, you’ll be guided through:

• Finalizing your Fall schedule
• Completing health requirements clearance
• Submitting any necessary payments (if applicable)
